Plan Details:
• 15% discount applied when payments due are paid in full at the time of service using cash or check
• 10% discount applied when payments due are paid in full at the time of service using credit cards
• 12-month membership fee is due in full upon joining.
• It is your responsibility to maximize your benefits by arranging all appropriate
appointments within the 12-month membership period.
• If benefits are not used, you are not entitled to a refund.
• Membership is non-transferable and can only be used at Topsham Dental Arts.
• Membership cannot be combined with any dental insurance coverage, office discounts or
payment plans (including Care Credit).
• Membership requires annual doctor exams and patient to remain current on radiographs.
• The Oral Health Membership Plan is not automatically renewed at the end of the benefit year.
